The IRS has released draft 1094 and 1095 forms for the 2017 reporting year. The forms have only minor changes thus far:
Form 1094-C: Box C in line 22 has been removed and is now labeled as “Reserved”.
Forms 1095-B and 1095-C: A paragraph has been added to the instructions which directs individuals to the IRS website to provide information on the individual and employer shared responsibility provisions and premium tax credits.
Copies of the draft forms can be found using the links below.
Please keep in mind the following:
- Forms 1094-C and 1095-C are completed by Applicable Large Employers (employers with 50+ full-time equivalent employees) that are both fully-insured and self-insured.
- Forms 1094-B and 1095-B are completed by employers with fewer than 50 full-time equivalent employees that are self-funded.
These forms will be completed in early 2018 for the 2017 calendar year. More information will be provided when the forms are finalized.
